Dr. Sohel M. Momin

Board Certified Internal Medicine

Dr. Momin obtained his medical degree at the Medical College of Georgia after completing a double major in chemistry and biology at Emory University. He performed his residency at Jackson Memorial Hospital in Miami, where he gained extensive medical experience in the area of acute medicine and became board certified in Internal Medicine in 2004. Northside Forsyth Internal Medicine & Geriatrics was established in October 2005 and shortly thereafter it developed into a thriving practice. Dr. Momin maintains professional relationships with a wide range of specialist in the area, serving the community and surrounding areas. Our mission has always been to develop and maintain a personal and trusting relationship with all our patients, providing them with the highest medical attention and care. Dr. Momin values his family time with his wife Madelin and their two daughters, who reside in Milton.

Dr. Briana Ford

Dr. Ford obtained her medical degree from Lincoln Memorial University DeBusk College of Osteopathic Medicine in 2022 and completed her internal medicine residency training at Northside Hospital Gwinnett in Lawrenceville, Georgia, where she gained extensive experience in managing complex and high-acuity patients. She has special interests in preventive health, lifestyle medicine, and patient safety, and has presented her work at regional and national conferences. Dr. Ford believes in working together with patients to create practical, personalized plans for long-term wellness. Joining our practice in August 2025, Dr. Ford works alongside Dr. Momin, developing trusting relationships with her patients while providing excellent care. Outside of medicine, Dr. Ford enjoys visiting local cafés, staying active, and spending time with her cats Rosie and Mina.

Megan Smith

Nurse Practitioner FNP-C

Megan graduated from the University of Georgia in 2012, where she majored in Biochemistry. She then earned her Bachelor of Science in Nursing from the Medical University of South Carolina in 2014, and her Doctorate in Family Nursing in 2019. She also gained extensive experience in Oncology working as an RN. In January of 2018, Megan did her Nurse Practitioner training under Dr. Momin and Tracy McDaniels. After graduating in May of 2019, the physicians brought her on board and she has developed strong patient relations and provides excellent care and has been a great addition to our practice Megan grew up in Cumming, enjoys spending time on Lake Lanier with her husband and daughter and is proud to be a Georgia alumnus, Go Dawgs!!

Jenn Faulkner

Nurse Practitioner FNP-C

Jenn graduated from the University of Georgia in 2015 with a degree in Biology. She then went into the accelerated nursing program at the University of South Alabama in 2017, earning a Bachelor of Science in Nursing. While working as a Registered Nurse in Oncology, she completed her Masters of Nursing at South Alabama, specializing in Family Nurse Practitioner. Jenn gained extensive experience at Piedmont Hospital Atlanta as a Registered Nurse, was the Promise 360 Award Winner, and was nominated for Nurse Preceptor of the Year. She completed her Nurse Practitioner training at our practice, under the guidance of Dr. Momin. Jenn joined our team in 2021. She lives in Dallas, GA with her husband and 2 daughters. She enjoys spending her free time with family and friends cheering on the Dawgs!

Jessica Hyland

Nurse Practitioner FNP-C

Jessica graduated from the University of Kentucky Magna Cum Laude as well as Dean's List with a Bachelor of Science in Dietetics. She earned a Master of Science in Nursing from Emory University in December 2023 with extensive clinical experience in cardiology, family medicine, endocrinology, and pediatrics. Throughout her time at Emory, she was a member of the Sigma Theta Tau Honors Society. Jessica joined our practice in July 2024 under Dr. Momin's direction, using her skill, knowledge, and love of patients to provide excellent care. Jessica is a native Georgia girl who enjoys spending her free time on the lake with family and friends, traveling, and staying active

Meghan Englishby

Nurse Practitioner FNP-C

Meghan earned both her Bachelor of Science in Nursing and her Doctor of Nursing Practice as a Family Nurse Practitioner from the University of Tennessee. She completed her nurse practitioner training at Dr. Momin’s office under the mentorship of Jenn. Prior to becoming a nurse practitioner, Meghan worked as a registered nurse in medical-surgical nursing, hospice care, urgent care, and pediatrics, giving her a well-rounded clinical background across the lifespan. Meghan lives in Cumming with her husband and daughter. She enjoys playing tennis and cheering on the Tennessee Volunteers.

Nicole Herod

ARNP

Nicole earned her Bachelor of Science in Nursing from Georgia Southern University and her Master of Science in Nursing as a Family Nurse Practitioner from the University of Alabama at Birmingham. She completed her nurse practitioner clinical training in pediatric, adult, and geriatric primary care. Prior to becoming a nurse practitioner, Nicole spent nearly six years as a registered nurse at Children’s Healthcare of Atlanta, specializing in pediatric intensive care and caring for critically ill infants and children. She is passionate about providing compassionate, patient-centered care and encouraging patients to take an active role in their health and wellness. Nicole enjoys spending time with her husband and dog, Rizzo. Some of Nicole’s hobbies include reading books, completing puzzles, and playing tennis with her family and friends.

Katharine Houlihan

ARNP

Kate holds a Master of Science in Nursing from Herzing University, after earning her Bachelor of Science from the University of Kentucky. She has over six years of experience as an Emergency Department Registered Nurse and completed her Nurse Practitioner training in Dr. Momin’s ofﬁce. Kate has a passion for preventative care is a great addition to our practice. She enjoys spending time with her family, traveling, and exploring new coffee shops!
