Dr. Sohel N. Momin
Board Certified Internal Medicine

Dr. Momin obtained his medical degree at the Medical College of Georgia after completing a double major in chemistry and biology at Emory University. He performed his residency at Jackson Memorial Hospital in Miami, where he gained extensive medical experience in the area of acute medicine and became board certified in Internal Medicine in 2004.
Along with his associate Dr. Hemen Padhiar, they opened Northside Forsyth Internal Medicine & Geriatrics in October of 2005. They quickly developed a thriving practice and added two nurse practitioners to the group. Dr. Momin maintains professional relationships with a wide range of specialist in the area.
Our mission has always been to develop and maintain a personal and trusting relationship with all our patients, providing them with the highest medical attention and care.
Dr. Momin values his family time with his wife Madelin and their two daughters, who reside in Milton.

Dr. Hemen Padhiar
Board Certified Family Practice

Dr. Padhiar studied abroad and earned his medical degree in Katowice, Poland at the University of Silesia in 2001. Upon returning to the states, he completed his three-year Family Practice residency at the Medical College of Georgia in 2004, one year in Augusta, Georgia and two years in Waycross, Georgia with a private practice group. During his residency training, Dr. Padhiar was exposed to a broad range of specialties that included clinic, hospital and nursing home care, which he has incorporated into his practice still today. He became board certified in Family Medicine in 2004
Dr. Padhiar has held the position of Medical Director at local nursing homes and continues his relationship with many Assisted Living Facilities. In caring for the geriatric community, and has enlisted Tracy McDaniels NP, to continue the on-site visits to ALF facilities. He is currently the medical director of the geriatric program at Northside Hospital in Cumming as well as with various home health agencies.
Dr. Padhiar resides in Cumming with his wife Amy and enjoys traveling and is an avid sport fan!

Tracy McDaniels
Nurse Practitioner
FNP-C

Tracy earned her BSN in 1993 from the University of Central Florida. She
graduated in 2007 from North Georgia University with her Master’s Degree in
Nursing-Family Nurse Practitioner. She has worked in various areas of nursing
throughout her career including ICU and ER. After becoming a Nurse Practitioner,
Tracy worked in Infectious Disease, as well as Pulmonary Critical Care before
joining Northside Forsyth Internal Medicine & Geriatrics in November of 2016.
Prior to joining the practice, Tracy attended to patients in several assisted living
facilities (ALF}, and continues her outreach in the surrounding area on a weekly
basis. Those patients who find it challenging to leave their residence are truly
grateful for the compassion she shows ‘and appreciation for her passion for our
geriatric community.
Tracy resides in Cumming along with her husband Rob, three children and their
two, four-legged children, who all enjoy traveling together.

Megan Bueltel
Nurse Practitioner
FNP-C
Megan graduated from the University of Georgia in 2012, where she majored in Biochemistry. She then earned her Bachelor of Science in Nursing from the Medical University of South Carolina in 2014, and her Doctorate in Family Nursing in 2019. She also gained extensive experience in Oncology working as an RN. In January of 2018, Megan did her Nurse Practitioner training under Dr. Momin and Tracy McDaniels.
After graduating in May of 2019, the physicians brought her on board and she has developed strong patient relations and provides excellent care and has been a great addition to our practice.
Megan grew up in Cumming, enjoys spending time on Lake Lanier and is proud to be a Georgia alumnus, Go Dawgs!!

Mēgan Popp
Nurse Practitioner
FNP-C

Mēgan graduated from Georgia State University with a Bachelor degree in Exercise Physiology in 2007. She was the program coordinator for Children’s Healthcare of Atlanta at Scottish Rite Hospital in Atlanta, developing programs for obese children. From then she earned her Bachelor’s degree in nursing from Piedmont College in 2012 and worked in the labor and delivery unit at Habersham Medical Center. Mēgan went on to earn her Master’s degree as a Family Nurse Practitioner from South University. She then secured an RN position in the labor and delivery department at Northside Hospital Forsyth, where she provided care to high-risk perinatal patients as well as a Childbirth Educator.
Mēgan trained in our office alongside Tracy McDaniels, one of our nurse practitioners, where she gained a great deal of knowledge, skill and understanding for the Geriatric population.
Mēgan is originally from Minnesota, but her heart is in the south! She lives with her husband and three children and is an avid distance runner.

Jenn Faulkner
Nurse Practitioner
FNP-C
Jenn graduated from the University of Georgia in 2015 with a degree in Biology. She then went into the accelerated nursing program at the University of South Alabama in 2017, earning a Bachelor of Science in Nursing. While working as a Registered Nurse in Oncology, she completed her Masters of Nursing at South Alabama, specializing in Family Nurse Practitioner.
Jennifer gained extensive experience at Piedmont Hospital Atlanta as a Registered Nurse, was the Promise 360 Award Winner, and was nominated for Nurse Preceptor of the Year. She completed her Nurse Practitioner training at our practice, under the guidance of Dr. Momin and Megan Bueltel, N.P.
Jenn and her husband live in Smyrna and enjoy spending time with their family and friends, cheering on the Dawgs!

Anna Bowers
FNP-C
Anna graduated from the University of Memphis with a Bachelor of Science in Nursing degree in 2015. She went on to work at Northside Forsyth’s Post Anesthesia Surgical Unit and graduated from the University of South Alabama in 2022 with her Masters in Nursing, specializing in Family Nurse Practitioner. Anna has extensive bedside nursing experience in oncology, cardiovascular intensive care, and surgical services along with being certified in critical care. During this time, she won a Daisy award for Nurse excellence in 2020!
Anna came to our office to work alongside Dr. Momin, to gain experience while studying for her state boards and is now fully certified.
Anna and her husband moved here from Memphis, TN in 2020 and have since created a home in Cumming, GA. She enjoys yoga and kayaking with her pup on Lake Lanier!
We have been a patient of Dr. Momin’s for 6 years and we’ve never had a better doctor. His attention to detail, quick response back to us, and use of his internet portal are outstanding. His network of reliable specialists continues to amaze us. He not only attends to me and my wife, but our live in parent who is 91. You would be very lucky to have Dr. Momin and his practice as your doctor.
